Because we are exclusively a "referral-only" practice,
your primary care physician or specialist has referred you to see us
about a pulmonary (lung) or sleep-related disorder.
We take great pride in providing state-of-the-art
pulmonary care to all of our patients.
We understand that this can be a very
anxiety-provoking time of your life and hope to make this process of
establishing with us as easy as possible.
You will usually be given an appointment after your
physician has contacted us. We strive to make appointments as
quickly and convenient as possible for you.
Once your appointment is scheduled, you will need to
fill out registration forms, past history forms, etc. that are common to
any medical practice. Please be prepared to arrive at least 20
minutes early to fill out the appropriate forms. If you download
our forms, be certain to bring them with you to the appointment,
otherwise you will need to fill them out again.
PLEASE REMEMBER TO BRING ALL OF YOUR MEDICATION WITH
YOU TO YOUR APPOINTMENT, INCLUDING ANY INHALERS, SPRAYS,
OVER-THE-COUNTER, HERBAL, OR OTHER ALTERNATIVE MEDICATIONS.
